Anoressia Nervosa, il trattamento farmacologico

L’Anoressia Nervosa (AN) è una grave patologia psichica che causa importanti danni a livello fisico, emotivo e funzionale. Il disturbo è caratterizzato da: rifiuto di mantenere un peso corporeo minimo per età e genere, paura patologica di ingrassare, disturbo nel modo in cui le forme del corpo sono percepite, negazione di malattia, oppure indebita influenza sull’autostima del peso e delle forme del corpo, ed amenorrea secondaria (APA 2000 ; Spettigue et al 2008 ; Berkman et al 2006).

L’AN ha la più alta percentuale di mortalità tra tutti i disturbi psichiatrici e le possibilità di trattare questo disturbo rimangono ancora oggi molto limitate (Støving et al 2008). Il trattamento dell’AN tipicamente comprende interventi medici, nutrizionali e psicologici. La terapia farmacologica è frequentemente utilizzata sebbene la letteratura specialistica sull’argomento sia estremamente limitata (Spettigue et al 2008).
Spesso gli approcci all’AN sono molto eterogenei e la terapia nutrizionale, farmacologica, comportamentale e psicoterapica sono utilizzate separatamente o in combinazione senza una sistematica separazione dei livelli di intervento e senza una solida evidenza scientifica a supporto.

Basandoci sulla letteratura esistente riguardo al trattamento farmacologico dell’AN possiamo distinguere almeno tre approcci differenti in base al loro razionale:
(1) il trattamento farmacologico mirato sulle patologie presenti in comorbidità con l’AN (depressione, disturbo ossessivo-compulsivo, psicosi, etc) che si basa sull’uso di antidepressivi, antiossessivi ed antipsicotici;
(2) il trattamento farmacologico mirato alla prevenzione delle conseguenze dell’AN (osteoporosi, amenorrea secondaria) che comporta l’uso di vitamina D, terapie ormonali, terapie con estroporgestinici;
(3) il trattamento teoricamente “mirato” alla psicopatologia nucleare della AN che, basandosi sulle teorizzate disfunzioni dei sistemi serotoninergici, dopaminergici o del sistema oressigeno/anoressigeno, si basa su inibitori della ricaptazione della serotonina, dopamino antagonisti, e ghrelina.
In una recente revisione della letteratura scientifica sull’argomento sono stati analizzati i lavori sulla farmacoterapia dell’AN che erano risultati numericamente scarsi, metodologicamente carenti e non univoci nei risultati. La maggioranza degli studi erano stati effettuati per periodi troppo brevi, su un numero limitato di soggetti (spesso eterogenei per stadio di malattia e condizione nutrizionale) e raramente erano dotati della potenza statistica adeguata per trarre conclusioni (Støving et al 2008, Berkman et al 2006). Il trattamento farmacologico dell’anoressia nervosa basato sull’evidenza non è quindi ancora possibile alla luce dei dati disponibili.
Si può ipotizzare che l’olanzapina ed anche altri antipsicotici tipici e atipici possano rappresentare una opzione terapeutica a livelli di peso particolarmente bassi. Non è invece ancora chiaro se gli inibitori della ricaptazione della serotonina possano o meno essere utili nel prevenire le ricadute dell’AN successivamente alla stabilizzazione del quadro acuto. (Couturier e lock 2007) Si deve registrare un crescente interesse per il ruolo di leptina, ghrelina, e degli endocannabinoidi nella terapia dell’anoressia ma al momento attuale i dati disponibili non consentono di ipotizzarne l’uso nella pratica clinica.
